Dr Moore's consulting rooms are based at Calvary North Adelaide Hospital. Consulting days are Monday afternoons (monthly), Wednesday mornings (fortnightly) or Thursday afternoons (monthly).

Once a date has been scheduled, we will forward you a confirmation letter either via email or post, detailing all the relevant information. We will also send you a Patient Registration Form for you to complete and return to our rooms prior to your appointment.
All consultations require a valid referral letter. Referrals from General Practitioners are valid for 12 months whereas referrals from Specialists are valid for 3 months. Your referral's validity begins on the day of your initial consultation. We request all referral letters to be forwarded to our rooms prior to your appointment.
Consultation fees are payable in full on the day of your visit. We will submit a claim to Medicare on your behalf and you should receive your Medicare rebate into your nominated bank account within 24-48 hours. There is no rebate from private health insurance for consultation fees.
When attending your appointment with Dr Mark Moore, please bring any relevant pathology results and/or imaging studies where possible, or you may forward these to our rooms prior to your appointment.
If a surgical procedure is required, Dr Moore will discuss this with you at your appointment and you will be able to ask any questions you may have at this time. It is advisable to bring a list of questions with you. Following your consultation, a surgery date can be scheduled and you will be given an Informed Financial Consent with an explanation of fees and any expected out-of-pocket costs. If you require major surgery, you will require a second consultation with Dr Moore prior to your scheduled surgery.
If you require a procedure under Local Anaesthetic in the rooms, this will be scheduled on a separate day to your consultation. Dr Moore usually performs his Local Anaesthetic procedures on a Friday.
